We are looking for an advanced practice provider, specifically a nurse practitioner (NP) or physician assistant (PA), who has experience in primary care or internal medicine, ideally with a focus on geriatrics. This role offers the chance to enhance outcomes for residents in long-term care by collaborating with a dedicated team of providers and case managers.
If you are an advanced practice registered nurse (APRN or NP) or a physician assistant (PA) who is passionate about working with the elderly, we invite you to consider joining our team. Our providers primarily serve nursing homes or assisted living facilities, focusing on residents enrolled in specialized Medicare Advantage plans. Your responsibilities will include delivering personalized, coordinated healthcare that enhances quality of life and minimizes unnecessary hospital admissions.
Key Responsibilities- Utilize independent judgment to treat patients with both acute and chronic conditions, consulting with a supervising physician as necessary.
- Collaborate closely with facility partners and health plan leadership to implement our national care model while adhering to all federal and state regulations.
- Assess patients’ medical and mental health needs, developing care plans that proactively manage their conditions and address any barriers to care.
- Ensure compliance with local, state, and federal regulations regarding the clinical services provided.
- Monitor patients’ adherence to treatment plans and adjust as needed.
- Work alongside RN case managers as part of an integrated care team.
- Degree from an accredited APRN or PA program.
- A minimum of 3 years of clinical nursing or rehabilitation experience with geriatric populations.
- Experience with Electronic Health Records.
- Proficient in Microsoft applications, including Word, Outlook, and Excel.
- May be required to provide training and guidance to facility staff.
