What you'll do as a home health admit RN As a PRN admit nurse, your primary focus is new admissions and readmissions across the Fort Worth area, with occasional routine visits when the team needs extra coverage. Typically, your work includes: Completing thorough start-of-care and resumption-of-care assessments with OASIS documentation Establishing the initial plan of care with the physician Educating patients and families at the start of their care Coordinating with the clinical team for a smooth hand-off to ongoing visits Stepping in on routine visits when needed Documenting each visit accurately and on time Moreover, you work independently while a supportive team backs you up. What we are looking for First, you need an active Texas RN license.
