Program/Location: Plymouth Inpatient Campus - Section 35 Unit Education/Licensure: Associate's degree, High School diploma, or GED preferred Pay Range: $22-$27/hour (experience dependent) Status: Full Time

About Us

High Point & Affiliated Organizations is a health and human service agency whose mission is to treat and prevent substance use disorders and mental illness. High Point has programs located throughout Southeastern Massachusetts offering a full continuum of care for substance use and mental health treatment, including inpatient, outpatient, residential, and community-based services. Programs and services also assist survivors of abuse, violence, and families experiencing homelessness. High Point believes that everyone has inherent goodness, worth, and dignity. High Point's Plymouth Inpatient campus offers Section 35 services for individuals that have been civilly committed to Substance Use Treatment. These services also include acute treatment services (ATS-detoxification) and clinical stabilization services (CSS). These programs are staff secured units and not locked.

Family Coordinator Requirements

• Must present as courteous and professional at all times.
• Demonstrated flexibility and ability to perform multiple tasks.
• Must have excellent verbal and written communication skills, including the ability to organize ideas in logical and clear fashion.
• Knowledge of basic computer skills and software programs including the ability to learn, use and train High Point & Affiliated Organization's (HPAO) Electronic Heath Records (EHR).
• Must have the ability to work with others in a cooperative and collaborative manner.
• An understanding of the relationship between substance use and mental health issues and/or self-help programs.
• Understand and maintain therapeutic boundaries and reinforce rules in a respectful manner.
• Comply with Federal Regulations, 42 CFR Part 2, Confidentiality of Substance Use Disorder and HIPAA Regulations.
• TB/Flu and Covid-19 Assessment.
• Completion of the HPAO approved de-escalation program.
• Successful completion of all trainings upon hire and annually as required.

Family Coordinator Duties & Responsibilities:

• Maintains assigned caseload.
• Providing direct care to clients - including assessments, individual counseling, discharge and aftercare planning, treatment plans and updates, weekly progress notes, daily groups facilitation, group note completion, and completion of bio-psychosocial assessments.
• Making referrals when appropriate.
• Responsible for each assigned patients' medical records. All records are to be professional, complete and thorough with entries made in a timely manner.
• Responsible for linking patients with aftercare services prior to discharge and aiding in the patient and their family access services when appropriate.
• Strengthen current and create new partnerships within the community to ensure that the patients develop skills needed to sustain sobriety and/or housing.
• Review patients/family rights and responsibilities, emergency safety plans and grievance policy.
• Communicating/involving family in treatment at patient's request.
• Maintain professionalism when connecting with individual's collaterals.
• Attend and participate in all unit, facility meetings, case conferences, Treatment Team meetings, and mandatory trainings, as directed by the Clinical Director/Program Director/Site Director.
• Responsible for continued education in the field respective to their service level.
• All other duties as assigned by Clinical Director/Program Director/Site Director/COO.

Family Coordinator Duties and Responsibilities - Inpatient Services Specific:

• Responsible for contacting the insurance company for authorization, as needed.
• Utilize walkie-talkie as campus responders.
• Respond to codes as necessary.
• Coordination of aftercare services, appointments, and referrals.
• Facilitation of identified groups and milieu activities as assigned.
• Participation in MDT meetings to support patient care coordination.
• Assistance with monitoring patient observation checks, as needed.
• Completion of required documentation, including: consents/releases, Group Notes, Progress notes, Contact logs.
• Supporting patients in crisis through verbal de-escalation, 1:1 monitoring, or crisis/safety planning, as requested.
• Attendance in supervision and staff trainings.
• Request extensions after 90 days of treatment if needed.

Family Coordinator Qualifications:

• High School Diploma or GED, preferred.
• Associate's degree in human services, preferred.
• One-year experience in substance use treatment and/or mental health field.
• CPR Certification, preferred.
• Personal lived experience in mental health and/or substance use recovery.
• Experience working with families impacted by mental health and/or substance use recovery.
