This role is responsible for evaluation, treatment planning, team collaboration, and supervision of therapy staff to support residents’ recovery and mobility goals.### **Key Responsibilities*** Evaluate residents within required timeframes and develop individualized **treatment plans*** Provide therapy services in accordance with **physician orders and care goals*** Document evaluations, treatments, and progress notes per regulatory standards* Supervise **PTAs, aides, and students** in compliance with state practice act* Collaborate with interdisciplinary team members on resident care plans* Participate in **care conferences, utilization review, and rehab meetings*** Educate residents, families, and caregivers on treatment plans and discharge needs* Assist with **durable medical equipment (DME)** recommendations* Provide staff training and in-service education* Maintain compliance with all **state, federal, and payer regulations**### **Qualifications*** Degree in Physical Therapy (Bachelor’s required; Master’s or Doctorate preferred)* Active **South Carolina PT license** in good standing* Experience in **skilled nursing or rehab setting preferred*** Familiarity with **PointClickCare and Casamba** preferred* Strong clinical, leadership, and documentation skillsWe are an equal opportunity employer.
