Nurse Practitioner PRN

Job Responsibilities Patient Care Provides medical care, palliates, and manages terminal illness in compliance with the state Nurse Practice Act under the supervision of a collaborating physician. Completes history and physical examinations, orders and interprets diagnostic tests, diagnoses health conditions, and documents findings. Develops individualized plans of care, prescribes medications or treatments, obtains referrals, and coordinates hospice services. Completes hospice face‑to‑face visit requirements to assess eligibility and provides clinical information for recertification. Makes recommendations to the hospice medical director regarding patient eligibility for certification and recertification. Reviews, coordinates, and oversees patient care in consultation with the interdisciplinary team. Ensures symptom control measures are delivered through collaboration with team members. Reviews protocols and proposes interventions based on literature in collaboration with the physician. Consults the collaborating physician when the plan of care is outside standard practice. Communication Collaborates with the interdisciplinary group to create, review, and revise the patient's plan of care. Consults with attending physicians, hospice medical director, and other practitioners about eligibility and medical management. Coordinates with community health practitioners. Attends and participates in interdisciplinary group meetings. Completes and submits accurate clinical documentation. Educates patients, families, and caregivers about preventive care, medical and psychological issues, and spirituality. Provides a safe environment for the patient. Supports family/caregiver continuity of care. Identifies goals of care and meets needs with the interdisciplinary team. Serves as a nursing resource for education and consultation. Builds relationships with staff and community referral sources to facilitate program growth. Additional Duties Maintains knowledge of Medicare/Medicaid, state and federal hospice regulations. Ensures compliance with Medicare conditions of participation and other regulations. Complies with HIPAA requirements. Participates in quality monitoring and improvement initiatives. Pursues professional growth through seminars, conferences, and continuing education. Fulfills assigned or accepted assignments. Demonstrates knowledge in communication and counseling patients and families about end‑of‑life issues. Job Qualifications Graduate of an accredited School of Nursing and Nurse Practitioner Program. Current DEA license. Current state nursing licensure and CPR certification. Master's degree with at least one year of Nurse Practitioner experience and one year of hospice or palliative care experience. Accreditation from an approved certifying body for advanced practice nursing (e.g., CHPN, Pediatrics, or Geriatrics). Excellent observation, verbal and written communication, problem‑solving, and mathematical skills. Physical ability to walk, stand, lift, transfer patients, and perform required tasks. Licensed driver with a reliable automobile. Physical Demands The employee must frequently sit or stand and occasionally lift or move up to 50 pounds.
