Work Environment

This position requires an onsite presence as you will be engaged in home care services. Key Responsibilities

• Assist licensed Physical Therapists in conducting comprehensive evaluations and assessments of patients' physical conditions, ensuring each interaction is conducted with respect and empathy.
• Implement and modify treatment plans as directed by the Physical Therapist, tailoring exercises and therapy techniques to meet individual patient needs and maximizing rehabilitation outcomes.
• Instruct and monitor patients on their therapeutic exercises, ensuring proper techniques are used while providing encouragement and motivation throughout their treatment journey.
• Utilize evidence-based modalities such as heat therapy, cold packs, and electrical stimulation to alleviate pain and enhance functional abilities, all while ensuring the safety and well-being of each patient.
• Educate patients and their families regarding home exercise programs, ensuring they understand the importance of their rehabilitation and how to integrate these techniques into daily life.
• Thoroughly document patient progress and treatment outcomes, maintaining accurate records to support continuity of care and treatment planning.
• Collaborate with a range of healthcare professionals, including physicians and social workers, to optimize patient care and ensure seamless communication throughout the treatment process.
• Maintain a high level of professionalism and adhere to all regulatory and ethical standards in patient care and documentation practices.

Patient Population / Client Focus

Qualifications

• Completion of an Associate's degree from an accredited Physical Therapy Assistant program.
