Medical Social Worker (MSW) – Home Health Company: BAYADA Home Health Care Location: Roxboro, NC Job Type: Full-time (per diem opportunity on a per visit basis) We are seeking an energetic and experienced Licensed Medical Social Worker, MSW, to perform home health visits in designated territories. The role is integral to a multi-disciplinary team providing skilled nursing and rehabilitative care. Responsibilities Make home visits to clients in designated geographic territories. Assist the team with understanding social and emotional factors related to clients' health problems. Provide clinical social evaluations and plan appropriate interventions based on evaluation findings. Identify community resource referrals to address practical and environmental needs. Act as advocate for clients and families navigating the community system. Participate in the development of the total plan of care with the multidisciplinary team. Develop and maintain contracts with public and private agencies. Educate and instruct patients, families, or other representatives about community resources. Accurately document observations, interventions, and evaluations using a touch‑pad tablet. Qualifications Licensed by the STATE Board of Social Work Examiners at the Master (LMSW) or Independent (LISW) level. Minimum of a Master’s in Social Work with at least 2 years of community-based experience. Accreditation by the Council on Social Work Education (transcript or degree). Home care experience a plus. Excellent written and verbal communication in English. Ability to work independently and manage time effectively. Strong interpersonal skills. Solid computer skills; prior experience with electronic medical records preferred. Ability to travel to cases as assigned.
