LTSS Service Coordinator - RN Clinician

Job Title: LTSS Service Coordinator – RN Clinician Location: Must reside in San Jacinto, Montgomery, or Trinity counties, Texas, and be willing to travel up to 1 hour daily. Primarily field‑based work, with occasional office attendance. Responsibilities Manage member cases within the scope of RN licensure. Perform face‑to‑face clinical assessments to identify, evaluate, coordinate, and manage member needs across physical, behavioral, social services, and long‑term services/supports. Develop, monitor, evaluate, and revise individual care plans to optimize member health care across the continuum. Identify high‑risk members and coordinate care with members and the health‑care team, ensuring cost‑effective utilization of benefits. Facilitate authorizations/referrals and assist in implementing care plans as needed. Supervise non‑RN clinicians (LPN/LVN, LSW, LCSW, LMSW) by assigning tasks, verifying member information, conducting assessments, and providing performance feedback. Interface with Medical Directors, Physician Advisors, and Inter‑Disciplinary Teams on treatment plans; assist in problem‑solving with providers, claims, or service issues. Qualifications Registered Nurse with a current, unrestricted license in the applicable Texas county. Minimum 3 years of experience as a Service Coordinator, Case Manager, or similar role with individuals who have chronic illnesses, co‑morbidities, and/or disabilities. Knowledge of state law and contract requirements for RN supervision of non‑RN clinicians. Strong communication, organizational, and care‑plan development skills. Ability to travel up to 1 hour daily and work a field‑based schedule. Preferred MA or MS in Health or Nursing. State‑specified certifications as required by law or contract.
