Job Summary Homeside Rehab is seeking a dedicated and detail-oriented Occupational Therapist or OTA to join our home health team. In this role, you will evaluate patients, develop individualized treatment plans, and provide evidence-based OT interventions to improve motor skills, functional independence, and developmental outcomes. Job Responsibilities Evaluate patients and develop individualized treatment plans based on clinical assessment and patient goals Provide evidence-based OT interventions to improve motor skills, functional independence, and developmental outcomes Maintain accurate, timely documentation of all patient care in accordance with regulatory requirements Collaborate with the clinical team to ensure coordinated and effective treatment Communicate clearly with office staff, patients, and caregivers regarding progress and care plans Job Qualifications Active NY Occupational Therapy License (OTR/L) or OTA License Basic computer proficiency for electronic documentation Strong clinical assessment and interpersonal communication skills Ability to work independently in a home health environment Valid NY driver's license and reliable transportation for home visits Minimum 1 year of experience in Home Health, Outpatient, or Skilled Nursing Facility (SNF) settings Job Details Job Type: Full-time, Part-time, Per Diem Location: Swan Lake, Fallsburg, Woodridge & Monticello, NY Salary: $45 - $70 HOURLY Schedule Flexibility: Choose your own hours, day and evening shifts available, Monday to Friday, with weekend availability as needed Benefits Flexible schedule Referral bonus Therapist of the Month bonus License/Certification New York State Occupational Therapy License (Required) How to Apply If you are a motivated and compassionate Occupational Therapist or OTA, we encourage you to apply for this rewarding opportunity.
