Ambulatory Care Pharmacist: MTM & Outcomes Leader

Summit Health in Florham Park, NJ is seeking an Ambulatory Care Clinical Pharmacist to provide MTM and collaborative patient care in an outpatient setting. The role emphasizes evidence-based therapy decisions, patient education, and cost-effective drug management across chronic diseases.
