Registered Nurse Case ManagerPatriot Hospice is a rapidly growing, quality focused, family owned and operated local home health hospice network looking for a Registered Nurse Case Manager to join our team! Territory: We are looking to fill roles in Mahoning, Trumbull, and Columbiana counties. Business hours: Monday - Friday, 8:00 AM to 4:30 PM. Schedule for this position: 4 10 hour days - Monday-Thursday 9am-7pm. We recognize family as the cornerstone of our company, and want you to be part of a team that values your contributions and well-being.We value our families and community:On call not mandatedOne holiday per yearNo weekendsFlexible schedulePaid time offBurnout timeHardship program- program to support staff during challenging timesPatriot Cares- nonprofit for community outreachHow we support you professionally:This position is paid at a competitive hourly rate, with additional earning potential available through opportunities to pick up visits at our Per Diem rateCase loads of 12-15 patientsMileage reimbursement for any company travelMedical/Dental/Vision insurance/Supplemental insurance (begins immediately upon hire)Company 401K with 4% company matchingAs a RN Admission Nurse, you will:Have coordination of total patient careEvaluate the effectiveness of nursing service to the patient and familyPrepare and present patient's record to the Clinical Record Review Committee as indicatedParticipate in the patient's plan of care during the admission processAll other duties as assignedQualifications:Must be a graduate from an accredited School of NursingMust be licensed in the State as a Registered NurseOne or more years of experience in community/home health agency or hospital preferredMust have knowledge of Medicare and Medicaid guidelinesMust have a criminal background checkMust have a current BLS CPR certificationJob conditions:Must have a driver's license and be willing and able to drive to patient's residencesThe ability to access patients' homes which may not be routinely wheelchair accessible is required. Hearing, eyesight, and physical dexterity must be sufficient to perform a physical assessment of the patient's condition and to perform and demonstrate patient carePhysical activities will include walking, sitting, stooping, and standing and minimal to maximum lifting of patients and turning of patients*This position is paid at a competitive hourly rate with the opportunity to earn extra money by picking up visits at our Per Diem rate*We cannot wait for you to join our team!
