Job Summary We are hiring a Primary Care Nurse Practitioner / Physician Assistant (NP/PA) to provide on‑site primary care to residents in long‑term care settings. This full‑time role is ideal for a provider who values meaningful patient relationships and thrives in a collaborative care environment.
