Clinical Team Manager, RN - Home Health
Full Time | Port St Lucie, FL
Trilogy Home Healthcare , a Humana company, is hiring a dedicated and compassionate Clinical Team Manager, Registered Nurse (RN) to join our award-winning team. What You'll Do:
Oversee day-to day operations of a clinical interdisciplinary team
Ensure quality and appropriate care delivery and staffing
Act as liaison to resolve patient care issues and communicate with physicians
Supervise, mentor, and train clinical staff in collaboration with other leaders
Support clinical program oversight and ensure compliance with agency goals

What You Bring:
Graduate of an accredited nursing program; must be a licensed Registered Nurse, RN
1+ year of Medicare Home Health field experience; Experienced field Registered Nurses encouraged to apply!
Strong judgement, leadership, and problem-solving skills
