Position Title: Registered Nurse (RN) - Home Health

Shift: Day 5x8-Hour (08:00 - 17:00)

Job Description

We are seeking a dedicated Registered Nurse (RN) for a Home Health position that involves coordinating and directing home nursing care in accordance with physician orders, agency policies, and procedures. The RN will apply the nursing process competently and will be responsible for supervising and evaluating Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.

Key Responsibilities

• Provide nursing care in patients' homes, based on assessment and established care plans.
• Conduct Start of Care, Wound Care, Follow-up Visits, and other nursing evaluations.
• Ensure compliance with documentation practices and complete all required paperwork within 24 hours of patient visits.
• Maintain effective communication with the healthcare team and families for coordinated patient care.
• Supervise and evaluate the performance of Home Health Aides and LPNs/LVNs.
• Participate in periodic office meetings.

Qualifications

• Minimum of 1 year of recent Adult Home Health Experience or Hospice experience, depending on specialty.
• Must hold an active nursing license.
• Current CPR and BLS certification required.
• Proficient in HomeCare HomeBase (HCHB) and OASIS documentation.
• Reliable transportation, valid driver's license, and agency-required liability insurance are mandatory.

Travel Requirements

The RN will be required to travel to patient homes, which may involve driving up to 75 miles from the assigned clinic. Mileage will be reimbursed at the IRS standard rate.

Work Schedule

The typical schedule is Monday through Friday from 8 AM to 5 PM. Weekend work may be required based on location and patient needs.

Patient Load

On average, the RN will be responsible for 5-10 patients per day, equating to approximately 30 productivity points per week. Each type of visit and activity (including documentation and travel) holds a different point value.

Additional Information

This position does not guarantee hours, and the RN must be comfortable accepting all patients assigned to their schedule. All overtime must be pre-approved, and there is no assurance of a specific number of hours per week.

This position is based in Sunset Hills, MO.
