Hospice Admissions RN Dependable Hospice | Tucson, AZ & Surrounding Areas Full-Time | Monday–Friday | 8:00 AM–5:00 PM Be the Compassionate First Step in a Family’s Hospice Journey Dependable Hospice is seeking an experienced and compassionate Hospice Admissions RN to join our Tucson team. This nurse plays an essential role in helping patients and families transition into hospice care with understanding, dignity, and support. As an Admissions RN, you’ll meet with patients and families, complete clinical assessments, explain hospice services and benefits, and coordinate with physicians, referral sources, and our interdisciplinary team to ensure a smooth and timely admission. Competitive Compensation Medical, Dental & Vision Benefits Paid Time Off (PTO) 401(k) Monday–Friday Schedule Supportive, Experienced Leadership Collaborative Interdisciplinary Team Meaningful Work Supporting Patients and Families What You’ll Do Conduct comprehensive admission assessments in patients’ homes and other care settings Evaluate patients for hospice eligibility and identify clinical needs Educate patients and families about hospice philosophy, services, benefits, and what to expect Review medical history, diagnoses, medications, and current care needs Coordinate with physicians, hospitals, facilities, referral sources, and hospice team members Complete admission documentation accurately and within required timeframes Develop initial plans of care based on patient needs and goals Provide education regarding symptom management, medications, equipment, and end-of-life care Ensure necessary medications, supplies, and durable medical equipment are coordinated for admission Provide compassionate emotional support to patients and families Communicate important clinical information to the interdisciplinary hospice team Maintain compliance with hospice policies, CMS requirements, and applicable regulations What We’re Looking For Current, unrestricted Arizona Registered Nurse (RN) license Minimum 2 years of clinical nursing experience Hospice, home health, palliative care, or community-based nursing experience strongly preferred Knowledge of hospice eligibility and CMS guidelines preferred Strong clinical assessment and critical-thinking skills Excellent communication and patient/family education skills Compassionate and comfortable supporting patients and families during end-of-life care Strong organization, documentation, and time-management skills Experience with electronic medical records (EMR) Valid driver’s license and reliable transportation Schedule Full-Time | Monday–Friday | 8:00 AM–5:00 PM This is a field-based position serving patients throughout Tucson and surrounding areas , with travel to patient homes, facilities, and other care settings as needed.
