What You'll Do as a Case Manager Assess client needs and gather essential background information Develop individualized care plans and connect clients to appropriate resources Collaborate with therapists, medical providers, and other team members Provide ongoing support, follow-up, and coordination for your caseload Build strong, supportive relationships with clients and families Maintain clear documentation and contribute to a positive clinic environment Qualifications Applicants must meet ONE of the following: Bachelor's degree with Social Service Worker (SSW) license Bachelor's degree in any field with willingness to pursue SSW licensure Additional Requirements: Valid driver's license Basic understanding of Social Work principles Familiarity with social welfare policy and human development Ability to problem-solve and maintain a client-centered approach Spanish speakers may qualify for increased wages after testing Apply Today Ready to make a real impact?
