Community Health Navigator & Care Coordinator

InnovaCare Management Services Company, LLC

InnovaCare Management Services Company, LLC in Orlando, FL seeks a Community Health Worker (CHW) to support care coordination for patients by connecting them with social services and resources in the community. This role focuses on biopsychosocial assessments and linking patients to needed services.

The CHW will perform home visits and telephonic outreach, educate on advance directives, and document encounters to monitor progress and outcomes. Travel to clinics and community sites is expected.
