Speech Language Pathologist (SLP) - PRN/Per Diem Aspire Senior Living Carthage, a premier resident-focused care provider, is seeking a dynamic and compassionate Speech Language Pathologist (SLP) to join our exceptional team! As a PRN/Per Diem SLP, you will have the opportunity to provide high-quality patient care to our residents in a skilled nursing and rehabilitation setting. Responsibilities Assess referred patients that require rehabilitation services. Provide direct patient care to assess their medical condition, functional capabilities, limitations, restrictions, and potential for rehabilitation. Establish and administer a treatment program with specific goals determined according to the patient's capacity and tolerance under the direction of the Physician. Set realistic and achievable goals for patients. Document and record patients' conditions. Educate patients and families in an appropriate therapy method. Evaluate, write weekly progress notes, recertification, and discharge documentation. Collaborate with all disciplines to provide best patient-centered treatment and care. Perform all other duties assigned by Director of Rehabilitation. Maintain confidentiality, HIPAA, and Federal Residents' Rights. Qualifications / Requirements Must have graduated from an accredited Speech Therapy college or university. Must have a valid professional license and registration as a SLP in Missouri. Experience with long-term care and/or geriatric care is preferred.
