1st Choice is seeking Licensed Registered Nurses (RNs) to join our Nurse Monitoring Team supporting participants in Baltimore City, MD.

Licensed Registered Nurse (RN) – Nurse Monitor

Baltimore City, MD

1st Choice is hiring  Licensed RNs to provide  Nurse Monitoring services for adults who are chronically disabled or functionally impaired. This role includes in-home visits, care plan review, and ensuring participants receive the appropriate level of care.

Key Responsibilities

• Conduct in-home assessments and complete Nurse Monitor questionnaires
• Evaluate care interventions and recommend updates
• Identify service needs and reportable events
• Coordinate care with providers and community resources
• Advocate for participant rights and access to services
• Maintain accurate, timely documentation

Requirements

• Active  Maryland RN license
• 2 years medical-surgical, geriatric, pediatric, or case management experience
• 1 year community-based healthcare experience preferred (training provided)
• Must pass a background check

Schedule & Compensation

• Flexible 30 or more hours/week
• Paid  only for approved billable activities (visits, documentation, care coordination)
• Compensation based on  actual time spent on each billable task

Candidates must provide:

• TB Screening (within past 12 months – skin test or Quantiferon; chest X-ray if positive)
