Job Description

Job Summary:
Provide comprehensive physical therapy evaluations and treatments across inpatient and outpatient settings. Deliver patient-centered care with a focus on mobility, discharge planning, and functional recovery in diverse clinical environments.

Responsibilities:

• Conduct thorough PT evaluations for a wide range of diagnoses and health conditions.
• Develop and implement individualized treatment plans to improve patient mobility and function.
• Assist patients with mobility progression from acute care through SNF, ensuring safe transitions.
• Work effectively in emergency, ICU, Med/Surg, and outpatient clinics, including OB when needed.
• Document patient progress and collaborate with interdisciplinary teams to optimize outcomes.
• Manage a diverse caseload, seeing between 5 and 15 patients daily with appointments every 30-45 minutes.

Qualifications:

• Hold an active Hawaii PT license or be able to obtain licensure immediately.
• Proven experience performing PT evaluations and treatments in acute, outpatient, and inpatient settings.
• Demonstrate independence and thoroughness in rural healthcare environments.
• Possess strong knowledge of discharge planning and mobility progression protocols.
• Ability to work in fast-paced, diverse clinical settings including ER, ICU, and Med/Surg.
• Excellent communication and documentation skills.
