Job Title: Registered Nurse – Adult Care / Home Care

Location: Bronx, NY, United States
Job Type: Contract
Shift: Day Shift | Monday–Friday | 9:00 AM – 5:00 PM
Duration: Approximately 13 Weeks
Hours: 37.5–40 Hours per Week
Weekend Requirement: Every Other Weekend

"Navitas Healthcare, LLC" is seeking an experienced Registered Nurse to provide adult home care services throughout the Bronx. The RN will deliver individualized patient care for post-acute and medical-surgical patients in home settings while ensuring quality care, patient safety, and continuity of treatment.

• Provide direct nursing care to patients in home care settings
• Assess, plan, and implement individualized patient care plans
• Administer medications and monitor patient responses
• Perform wound care, IV therapy, and post-operative nursing care
• Monitor vital signs and report changes in patient condition
• Educate patients and families regarding treatment and recovery plans
• Maintain accurate documentation in electronic medical records
• Coordinate care and discharge planning with interdisciplinary teams
• Follow infection control and patient safety protocols
• Travel locally within the Bronx Borough to patient homes

• Active New York State RN License required
• Minimum 2 years of recent Med-Surg experience in acute or post-acute care
• AHA BLS Certification required
• Associate Degree in Nursing (minimum requirement)
• Strong skills in patient assessment, medication administration, IV therapy, and wound care
• Ability to work independently in home care environments

• Home care or visiting nurse experience preferred
• EPIC EMR experience preferred
• Knowledge of OASIS documentation preferred
• ACLS Certification preferred
