Description

• Conduct thorough assessments of patients within both inpatient and outpatient settings, crafting tailored therapy plans to meet diverse needs.
• Implement targeted physical therapy interventions concentrating on enhancing mobility, strength, balance, and overall function.
• Collaborate closely with nurses, physicians, case managers, and fellow rehabilitation professionals to promote optimal patient outcomes.
• Educate and empower patients and their families about therapeutic objectives, discharge planning, and recommended home exercise routines.
• Document comprehensive evaluations, treatment sessions, and patient progress in line with established facility and regulatory protocols.
• Engage in multidisciplinary care team rounds and interdisciplinary conferences to ensure a cohesive approach to patient care.

Education

• DPT (Doctorate in Physical Therapy) or an equivalent qualification from an accredited institution.

Certifications

• Active state licensure as a Physical Therapist
• Preferred: Current CPR certification
• Specialty certifications (e.g., neurological, geriatric) are advantageous but not essential.
