External Client Care Coordinator

to help connect families with quality in-home care while supporting an outstanding client experience. What You'll Do

Build strong relationships with clients, families, caregivers, and referral partners Conduct in-home consultations to understand client needs and recommend services Coordinate care plans and caregiver schedules to ensure quality service Follow up with clients to maintain satisfaction and address concerns Collaborate with office staff to provide seamless care coordination Maintain accurate client records and required documentation Develop partnerships with healthcare providers and community organizations Support local networking events and community outreach initiatives Help identify opportunities to improve the client experience What We're Looking For
