Position Summary

As an LPN at Countryside Post Acute in Buchanan, you will deliver dependable nursing care that supports resident comfort, safety, and clinical outcomes. This role exists to ensure timely, accurate bedside care and clear communication across the care team.

Key responsibilities include medication administration and treatments, focused assessments and documentation, monitoring for changes in condition, supporting care plans, and coordinating with RNs, providers, and families.

Required qualifications/skills:
- Active Georgia LPN license in good standing
- Long-term care or skilled nursing experience preferred
- Strong documentation, organization, and communication skills
- Commitment to patient safety and teamwork

A Typical Shift

You’ll start by reviewing the unit’s overnight updates and priorities, then move through resident check-ins while coordinating with the interdisciplinary team. Throughout the day, you’ll balance planned tasks with real-time needs, communicate status changes promptly, and keep records current so transitions are smooth. You’ll close out the shift with clear handoff notes and a structured report to support continuity of care.
