Trinity Rehab is seeking a dedicated and compassionate Physical Therapist (PT) to join our team in Albemarle, NC on a PRN basis. Position Overview

A Physical Therapist helps injured or ill individuals improve movement, manage pain, and restore physical function. PTs are movement experts who evaluate patients, develop individualized treatment plans, and guide them through therapeutic exercises and interventions to achieve optimal outcomes.

Key Responsibilities

• Evaluate patients and develop individualized treatment plans
  
• Implement therapy interventions to improve mobility, strength, and function
  
• Monitor patient progress and adjust treatment plans as needed
  
• Collaborate with interdisciplinary team members to ensure quality care
  
• Educate patients and caregivers on treatment plans and home exercise programs
  
• Maintain accurate and timely documentation

Requirements

• Licensed Physical Therapist in the state of North Carolina
  
• Strong clinical skills in a SNF or rehab setting preferred
  
• Excellent communication and teamwork abilities
