Senior Market Physician Executive - In-Home Care Leader

Monogram Health seeks a Market Physician Executive (MPE) to lead in-home multi-specialty care within an assigned market, coordinating with APPs, RNs, LCSWs, and PharmDs to implement evidence-based clinical pathways.
