New York Community Care is seeking a compassionate Care Manager to join our growing Assisted Living Center. The ideal candidate will coordinate client care services, support patients and families, and ensure high-quality home care delivery while maintaining compliance with agency and state guidelines.

Responsibilities

• Coordinate and manage home care services for assigned clients

• Conduct client assessments and develop individualized care plans

• Communicate with patients, families, caregivers, and healthcare providers

• Schedule and oversee caregiver assignments and service hours

• Monitor client progress and update care plans as needed

• Ensure documentation is accurate, timely, and compliant

• Address client concerns and provide ongoing support

• Maintain knowledge of home care regulations and agency policies
