Position Overview

We are seeking a Nurse Practitioner who enjoys building relationships with their patients. In this role, you will work closely with our interdisciplinary care team to ensure comprehensive primary care. The position is based onsite in Surprise, Arizona and reports to the Associate Medical Director.

Key Responsibilities

You will provide comprehensive, primary care to our older adult population.

You will manage patient care, including both acute and chronic care management, prevention, and patient education.

Required Qualifications

Active, unrestricted nurse practitioner license in the state of Arizona

Board certification.

Proficiency with electronic medical records (EMR).

2 years of Experience working with Geriatrics

2 years of Primary Care Experience

Pay Range

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.

$123,800 - $170,300 per year

This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
