Key Responsibilities

As an LCSW working with our Hospice Center, your responsibilities will include:

  • Conducting thorough clinical assessments to evaluate the psychological, social, and emotional needs of patients and their families.
  • Developing individualized treatment plans based on evidence-based practices that address the complex dynamics of life-limiting illnesses.
  • Providing psychotherapy using effective modalities such as cognitive-behavioral therapy (CBT) and solution-focused therapy tailored to the needs of patients and families.
  • Maintaining accurate and timely documentation of patient records in compliance with HIPAA regulations, ensuring confidentiality and professional standards.
  • Partnering with an interdisciplinary team that includes physicians, nurses, and other health care professionals to offer holistic care and support for patients.
  • Engaging in case consultations and peer review meetings to share insights and enhance the quality of service delivered.
  • Educating patients and families about the psychosocial aspects of their conditions and available community resources to empower them in their care journey.
  • Staying informed about advancements in the field of social work and hospice care to ensure best practices are integrated into your daily responsibilities.

Patient Population / Client Focus

Your primary focus will be on hospice patients with terminal diagnoses and their families. This role requires a deep understanding of the emotional, psychological, and spiritual needs that arise during end-of-life care, as well as the ability to foster resilience and coping strategies in those facing grief and loss. Your expertise will help families navigate these challenges, providing them with essential support and guidance.

Qualifications

To ensure success in this position, candidates must meet the following qualifications:

  • A Master’s degree in Social Work from an accredited institution.
  • A valid Licensed Clinical Social Worker (LCSW) license to practice in Texas.
  • A minimum of three years of clinical experience, ideally within hospice or palliative care settings.
  • A robust understanding of solution-based therapeutic interventions that are adaptable to individual patients' needs.
  • Strong communication and interpersonal skills that facilitate effective patient interactions and collaboration with a multidisciplinary team.
  • Exceptional problem-solving and critical thinking capabilities, enabling you to handle complex cases with compassion and professionalism.

Preferred Experience

While not mandatory, candidates with experience in hospice care, grief counseling, or similar settings will find their skills particularly valuable in this role. Familiarity with case management software and teletherapy methodologies will also enhance your capacity to serve our patient population effectively.

Compensation and Benefits

This position offers a competitive salary range of USD 80,000 to 100,000 annually, commensurate with your experience and qualifications.
