Position Summary

The Family and Community Well-Being Coordinator supports the planning, implementation, evaluation, and continuous improvement of the Cultivate Wellness and Healthy Communities programs.

The Cultivate Wellness program is a free, at-home gardening and health program that empowers families to grow fresh, culturally relevant produce while increasing access to affordable, healthy food. Through hands-on gardening education and weekly home visits, staff support participating households with garden development, health education, service referrals, and personalized guidance that promotes healthier lifestyles and long-term well-being.

The Healthy Communities program promotes community well-being through health education, preventive services, and community-based events delivered in partnership with local healthcare organizations. The program connects individuals and families with culturally responsive education, enrichment opportunities, health screenings, and other on-site services that support improved health and access to care.

The Family and Community Well-Being Coordinator works directly with participants, community partners, and LCM staff to deliver culturally responsive programming that promotes health, well-being, self-sufficiency, and access to community resources. The position supports participant recruitment and engagement, program activities and events, data collection and reporting, partnership development, and health and wellness education. The coordinator also helps identify and address social determinants of health by connecting participants with appropriate resources, services, and community support.

• Demonstrate a strong commitment to advancing public health, community well-being, and efforts to address social determinants of health.

• Advocate for Latino and immigrant communities and help connect individuals and families with health, social service, education, and workforce resources.

• Think creatively and resourcefully to identify solutions and help participants overcome barriers to services and opportunities.

• Collaborate effectively with participants, LCM staff, healthcare providers, community organizations, and other partners using a holistic, cross-sector approach.

• Demonstrate strong organizational skills and attention to detail while maintaining confidentiality, patience, professionalism, and respect for participants.

• Exercise sound judgment, initiative, adaptability, and strong decision-making skills in implementing programs and responding to participant and community needs.

Essential Duties and Responsibilities

Program Coordination & Support

• Coordinate day-to-day program operations, participant-facing activities, and logistical support to ensure effective program implementation.

• Coordinate the planning and logistics of Cultivate Wellness and Healthy Communities activities, including scheduling, room setup, supply ordering, participant communication, event preparation, implementation, and follow-up.

• Track enrollment, registration, and program attendance; follow up with participants to support ongoing engagement.

• Provide participants with timely and accurate program information.

• Support the use of virtual platforms, such as Zoom and Google Meet, to enhance program accessibility.

• Support achievement of Cultivate Wellness and Healthy Communities goals related to health, community well-being, and social determinants of health.

• Coordinate the delivery of existing and new health promotion programs and activities, including Cultivate Wellness, Bienvenido, Spanish CPR/First Aid/AED certification courses, health initiatives, and the Siembra Salud internship, while supporting related community partnerships.

• Support effective referrals that connect participants with social services, health care, and other resources that address social determinants of health.

• Deliver health and wellness education that supports program goals and community needs.

Participant Engagement & Recruitment

• Represent LCM at community and school events to engage and recruit parents and other adult community members for FCW programs and services.

• Guide prospective participants through enrollment processes and orientation.

• Support participants in goal setting and connection to services, including social services, health care, and workforce resources.

Community Partner Coordination

• Maintain strong communication with partner organizations, including facilitating partner meetings to support program alignment and collaborative problem-solving.

• Coordinate presentations, site visits, or field trips in collaboration with community partners.
