RN - Case Management

Registered Nurse Case Manager

This hybrid/blended role is responsible for the active management of patients as assigned and will serve as a clinic nurse as designated. The Registered Nurse will collaborate with medical providers and interdisciplinary team members to establish individualized medical goals that maximize patient healthcare outcomes.

Responsibilities

• Coordinate multi-disciplinary care conferences for high-risk or complex patients.
• Manage panel acute and chronic care needs, continuously reviewing EHR reminders and triggers.
• Participate in case management reviews with a multi-disciplinary team of providers.
• Advocate for the patient and family, fostering decision-making and independence.
• Facilitate the coordination of patient referrals, tracking progress and outcomes.
• Perform staff nursing duties including assessment, documentation, education, and medication administration.
• Maintain a clean, safe working environment and ensure compliance with safety and infection control policies.
• Participate in department and clinic team improvement efforts.
• Respond to emergency situations in a clinical setting.

Required Experience / Certifications / Licensure

• Active RN license in Alaska required.
• At least 1 year of case management experience within the last 2 years.
• Basic Life Support certification.
• Bachelor's degree in nursing preferred.
• National Case Management certification preferred.

Schedule

This position follows a Monday to Friday schedule during daytime hours.
