Executive Director of Community Health Clinical Programs, Nurse Practitioner About the Company Prominent healthcare & social services organization Industry Hospital & Health Care Type Non Profit About the Role The Company is in search of an Executive Director of Community Health Clinical Programs, a pivotal role that demands a mission-driven individual with a strong clinical background. The successful candidate will serve as the clinical leader for a multi-partner care network, driving the integration of medical providers, health plans, and community-based organizations. This executive-level position is responsible for shaping clinical strategy, designing and advancing clinical frameworks, and identifying new programmatic opportunities that focus on prevention, chronic disease management, and evidence-based interventions. The role also involves building and maintaining strong relationships with healthcare stakeholders and guiding innovation to improve outcomes and reduce disparities. Applicants must be Nurse Practitioners with an active New York State NP license, and a clinical background in primary care or community health is required. Familiarity with DSRIP and previous experience with PCHM are highly preferred. The ideal candidate will have a demonstrated understanding of value-based payment models, experience in evaluating program outcomes, and knowledge of regional demographics, community health needs, and health disparities. The role requires the ability to support advocacy, community engagement, and public-facing initiatives, as well as the capacity to collaborate effectively with executive leadership and cross-functional teams.
