Physical Therapist (PT) – Job DescriptionJob Purpose: Evaluate patients, develop individualized care plans, and provide or direct physical therapy interventions prescribed by the referring physician, while considering the patient's physical condition and age-related needs.Key Responsibilities:Evaluate patients to determine physical therapy needs.Develop individualized treatment plans and goals.Provide skilled physical therapy treatments and therapeutic interventions.Administer or direct appropriate physical agents and rehabilitation procedures.Monitor patient progress and modify treatment plans as needed.Provide therapy appropriate to the patient's age and physical condition.Work with physicians and other healthcare professionals as part of the rehabilitation team.Maintain appropriate patient documentation and treatment records.Provide rehabilitation services to patients in a geriatric and rehab population, depending on the specific assignment.Required Qualifications:Bachelor of Science in Physical TherapyActive New York State Physical Therapist (PT) licenseMaster's degree in Physical Therapy preferredCSCS (Certified Strength & Conditioning Specialist) preferred/required on the current Staff PT postingPrior experience is not required for the current Staff PT position; new graduates may be accepted with up to six months of on-the-job training.Previous experience in a rehabilitative medicine setting is preferred.
