Requirements

✅ Minimum 1 year of Medicare-certified Home Health experience
✅ Active Hawaii RN License
✅ Experience with Homecare Home Base (HCHB) and OASIS
✅ Valid driver's license, reliable vehicle, and auto insurance
✅ COVID-19 & Flu Vaccinations required (no exemptions accepted)

Preferred Qualifications

• BSN
• Case Management experience

Schedule

• Day Shift
• Rotating weekends (2–3 per month as needed)
• On-call approximately 6 shifts per month, including weekends
