Psychiatric Rehabilitation Services Coordinator (PRSC) PM Shift

Psychiatric Rehabilitation Services Coordinator (PRSC) PM Shift Full‑time, PM shift (3pm–11:30pm). Position located in Behavioral Health at Crestwood Terrace, an Intermediate Care Facility for adults 21+. Compensation up to $24/hr based on experience. Union representation applies per contract. Responsibilities Provide group and individual psychiatric rehabilitation therapy. Deliver case management services to adults with a history of psychiatric hospitalizations. Assist patients with long‑term care and stabilization goals. Collaborate with interdisciplinary team to develop and implement therapeutic services and interventions. Participate in Life Skills Training Program and other program initiatives. Qualifications Prior experience as a PRSC, psychiatric rehab, or related mental health professional in a licensed nursing home, intermediate care facility, or specialized mental health rehab facility (SMHRF) preferred. Bachelor’s degree in a human services field (sociology, special education, rehabilitation counseling, psychology, or related). Minimum of one year of supervised experience in mental health or human services setting. Experience with Point Click Care (PCC) is ideal but not required.
