RN Care Manager: Coordinated Family-Centered Care

You will work closely with Health Home Coordinators and community agencies, providing education and support to families while participating in clinical rounds. The ideal candidate must have a Bachelor’s Degree, a valid RN license, and at least two years of clinical experience in an acute care setting.
