What You'll Be Doing Provide skilled RN home visits throughout the St. Paul territory Complete comprehensive patient assessments Perform wound care, medication management, injections, and other skilled nursing procedures Develop and update individualized care plans Educate patients and caregivers on medications and disease management Communicate with physicians and interdisciplinary care teams Complete timely documentation using Homecare Homebase Monitor patient progress and coordinate ongoing care Who We're Looking For Active Minnesota Registered Nurse (RN) license Experience in home health, acute care, rehabilitation, long-term care, or skilled nursing Strong clinical assessment and decision-making skills Excellent communication and patient education abilities Self-motivated with strong organizational skills Reliable transportation for local travel throughout the St. Paul service area About Home Health Care, Inc.
