Responsibilities
- Conduct comprehensive speech-language pathology evaluations for adult residents, including assessment of communication, cognitive-communication, voice, and swallowing function.
- Develop and implement individualized treatment plans aimed at maintaining or improving communication abilities, safe swallowing, and cognitive function; adjust plans based on progress and clinical data.
- Provide direct therapy sessions and, when appropriate, educate and supervise caregivers or support staff to support continuity of care.
- Document evaluations, goals, progress notes, treatment plans, and discharge planning in the resident record per facility policy and regulatory requirements.
- Collaborate with the interdisciplinary team (nursing, dietitians, physicians, and other therapy staff) to coordinate care and support functional goals.
- Participate in care planning meetings and quality improvement activities to support resident outcomes and regulatory compliance.
