Key Responsibilities\n

• Develop and implement individualized treatment plans based on thorough clinical assessments and ongoing evaluations of client needs.

• Engage actively in team meetings, contributing insights that affect the overall treatment strategies for patients in hospice care.

• Consistently monitor and assess clients’ social environments and emotional wellbeing to ensure effective support systems are in place.

• Collaborate closely with family members and caregivers, developing strategies to navigate the complex emotional landscapes associated with end-of-life care.

• Work with external stakeholders, including school staff and healthcare providers, to create supportive frameworks for students and families impacted by terminal illness or loss.

Patient Population / Client Focus\n

Our client base primarily includes patients receiving hospice care, as well as their families who require emotional and psychosocial support during a critical period. The LCSW will address diverse client needs, which can include grief counseling, coping strategies, and access to community resources to ensure holistic support is provided

Qualifications\n

• Licensed Clinical Social Worker (LCSW) certification is required.

• Minimum of 2 years of professional experience in a social work setting, preferably in hospice, palliative care, or a related field.

• Outstanding verbal communication skills to effectively counsel patients and families.

• Strong analytical and problem-solving abilities to develop and adapt effective treatment plans.

Compensation and Benefits\n
