We are seeking an experienced Admissions Nurse (RN) who is passionate about improving lives, building meaningful community partnerships, and ensuring residents receive the right care at the right time. $94,000-$105,000 per year based on experience! As the Admissions Nurse, you will serve as both a clinical leader and community ambassador. You will be instrumental in helping residents and families navigate challenging healthcare transitions while collaborating with hospitals, physicians, case managers, and community providers to ensure a smooth and successful admission process. What you'll do: Review, evaluate, and process admission referrals in a timely manner. Conduct comprehensive nursing assessments for prospective and high‑risk admissions, including medication reconciliation. Ensure appropriate clinical placement and coordinate bed assignments based on resident needs and acuity. Collaborate with physicians, case managers, discharge planners, social workers, and healthcare providers to facilitate smooth transitions into the facility. Assist with admission orders, nursing assessments, care plans, and other clinical requirements. Verify and confirm all physician orders prior to admission. Minimum two (2) years of Long-Term Care (LTC) nursing experience. Experience in Community-Based Care, Care Coordination, Transitional Care, Case Management, or related healthcare settings. Strong understanding of skilled nursing operations and post-acute care services. Excellent clinical assessment and critical thinking skills. Exceptional communication, relationship‑building, and customer service abilities. Ability to work independently while collaborating effectively with interdisciplinary teams. Strong organizational and time‑management skills.
