Physical Therapist (PT)

Job Summary

We are seeking a dedicated and compassionate Physical Therapist to join our team. This role includes providing therapy services in both skilled nursing facilities and home health settings, with varying responsibilities each week.

Key Responsibilities

Provide skilled physical therapy services to patients in a variety of settings, primarily focusing on skilled nursing facilities and home health.
Supervise therapy assistants as needed.
Utilize Net Health and HCHB software to document patient care and track progress.
Work collaboratively with a therapy supervisor, clinical specialists, and directors for professional development and support.
Meet productivity expectations:
- Skilled Nursing Facility: 82% productivity.
- Home Health: 95% productivity, including drive and documentation time.

Qualifications

Minimum of 1 year of experience required.
Current BLS Certification required.
New graduates are welcome; training will be provided.

Scheduling and Work Details

Typical work week comprises Monday through Friday, with options for alternative schedules (Tuesday to Saturday or Sunday to Thursday).
Standard shift length is 8 hours, with some variability in daily time lengths based on need.
Guaranteed 40 hours of work per week.
Required travel between locations for home health services; mileage is billable between locations and from patient to patient in home health.

Important Notes

Expectations for driving time and documentation are factored into productivity metrics.
This position may require coverage in both skilled nursing facilities and home health settings, primarily in the Grand Island area but may include other regions as needed.
A $500 fee will be incurred if the clinician withdraws or resigns with less than 14 days notice.

Start Date
