Intensive Case Manager (Lead)

About the Role The Crisis Center of Comal County is seeking a Lead Intensive Case Manager to provide high-touch, trauma-informed support to individuals and families experiencing crisis, including survivors of domestic violence, sexual assault, and homelessness. In this role, you will focus on immediate safety, stabilization, and rapid response for high-risk clients. You will manage a small, intensive caseload, conduct comprehensive assessments, develop individualized service and safety plans, and connect clients with critical resources such as emergency housing, healthcare, legal advocacy, counseling, and limited financial support. You will also collaborate closely with shelter staff, housing teams, and clinical providers to ensure continuity of care. This position may include supervisory responsibilities, providing mentorship and guidance to other case management staff to ensure high-quality, trauma-informed services. What You’ll Do Serve as a crisis responder for CCCC’s 24-hour crisis hotline and support clients in immediate crisis (safety concerns, housing instability, mental health needs). Provide intensive, hands-on case management services to high-acuity clients with a low caseload (approximately 1:15) to ensure individualized care. Conduct comprehensive assessments focused on safety and crisis stabilization; develop and implement individualized service and safety plans. Maintain frequent client contact (5–10 contacts per week), including 6–7 hours of direct client contact and regular documentation. Coordinate emergency services and interventions as needed, and advocate for clients to access community resources and benefits. Maintain accurate, timely documentation, track outcomes related to client stabilization and program effectiveness, and ensure compliance with confidentiality standards and agency policies. Participate in case staffing, team meetings, and supervision; collaborate with shelter staff, housing teams, and clinical providers. As assigned, provide guidance and mentorship to case management staff, support onboarding and training, and assist with complex or high-risk cases. Who You Are You are deeply committed to survivor-centered, trauma-informed, and culturally responsive practice. You stay calm and focused in high-stress situations and can de-escalate crisis with empathy and professionalism. You are highly organized, communicate clearly, and can balance frequent client contact with thorough, timely documentation. You thrive both independently and as part of a multidisciplinary team. Additional Requirements Bachelor’s degree in Social Work, Psychology, Human Services, or a related field. Minimum of 2–3 years of experience in case management, crisis intervention, or related social services. Strong crisis intervention and de-escalation skills; ability to manage high-stress situations with professionalism and empathy. Proficiency with Microsoft Office and case management systems. Current, valid Texas driver’s license and proof of auto liability insurance. Ability to pass an extensive background check (including fingerprinting, drug screen, and driver’s record check). Must be available to work evenings, weekends, and holidays to ensure clients’ needs are met. Preferred Experience working with high-acuity populations, including survivors of domestic violence, sexual assault, or homelessness. Prior supervisory or leadership experience.
