Hospice RN Case Manager — Home Care Leader

Constellation Health Services seeks a compassionate Registered Nurse to care for hospice patients in their homes, assisted living facilities, and skilled nursing facilities across Newtown, Southbury, and Bethel, CT. You will perform skilled nursing tasks, assess needs, and participate in weekly interdisciplinary meetings. The role requires CT RN licensure, 1–2 years of RN experience (hospice preferred), and a commitment to compassionate, family-centered care with weekend/holiday coverage as #J-18808-Ljbffr Constellation Health Services
