Job Title - Registered Nurse
Unit - Case Manager
Duration - 13 Weeks with possibility of extension
Shift - Day 5x8-Hour (08:00 - 16:30)
Location - Atlanta, GA 30309
Position Summary :
The RN Case Manager is responsible for coordinating and managing patient care across the continuum of care to ensure optimal clinical outcomes, cost-effective treatment, and a positive patient experience. The RN Case Manager performs comprehensive assessments, develops individualized care plans, and collaborates with physicians, nurses, social workers, and other healthcare professionals to coordinate services and facilitate timely interventions.
Job Duties :
Conduct comprehensive patient assessments and evaluate medical, psychosocial, and discharge planning needs.
Develop, implement, and update individualized care plans based on patient needs and physician recommendations.
Coordinate care across the continuum by collaborating with physicians, nurses, social workers, therapists, and other healthcare professionals.
UNIT DETAILS : Ability to assess, evaluate and drive discharge plan. Coordinate services to the patient's appropriate level of care. Ensure that treatment plan is initiated, evaluated and re-evaluated daily.
Note : 3 years of RN Case Manager Experience Required
