Northwest Kidney Centers seeks a full-time day shift Home Program Nurse Care Manager (Home Dialysis Training Nurse) to support the Kirkland, Bellevue and Snoqualmie Ridge Kidney Centers in peritoneal dialysis. The Home Program Nurse Care Manager delivers and documents care for patients dialyzing at Northwest Kidney Centers and in their home, performing assessments, planning care, directing and evaluating patients and their home environment and clinical results, education and training as well as #J-18808-Ljbffr Northwest Kidney Centers
