Overview Social Worker — Behavioral Health | Travel assignment in a surgical center setting | Tuba City, AZ | Start 11/01/2026 | 13 weeks | $2,738–$2,864 per week | 36 guaranteed hours | Shift: 5x8 Rotating Key Highlights Weekly pay: $2,738–$2,864 per week Guaranteed hours: 36 hours per week Contract duration: 13 weeks Shift: 5x8 rotating Start date: 11/01/2026 Setting: Outpatient/Inpatient social work within a surgical center environment Housing: Shared contractor housing available; must apply 2 weeks prior; no pets Responsibilities Assess patients and develop care plans; coordinate a continuum of care to empower self-sufficiency Link patients to community, health care, substance use, and mental health resources; coordinate discharge planning and follow-up Review medical necessity for referrals to specialty services and collaborate with the care team to ensure appropriate resource use Communicate and coordinate with a multidisciplinary team to promote safe, efficient hospitalizations and smooth transitions Evaluate ED patients for care coordination needs and propose outpatient discharge plans with physician approval Manage care for patients with abuse, chronic pain, and frequent utilization, advocating for needed resources Requirements Education: Master’s degree in Social Work (MSW) Licensure/Certification: Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW); valid, current, unrestricted license in any state Certification: Basic Life Support (BLS) via American Heart Association Experience: Minimum three (3) years in a clinical care setting; experience with discharge planning, case management, or utilization review; knowledge of electronic health record systems Other: Ability to work in an inpatient/outpatient surgical center environment and participate in a travel assignment Benefits and Support Housing assistance: Shared contractor housing available; no pets; housing form due 2 weeks prior to start (via Medefis) 24/7 recruiter and travel support Fast credentialing and onboarding Extension opportunities Nationwide assignments Dedicated support throughout the assignment Orientation every 2 weeks About Healthforce Healthforce is a nationwide travel healthcare staffing agency connecting travel nurses and allied health professionals with high-quality contract assignments across the United States.
