Primary Care Physician (MD/DO)

We are looking for a compassionate and dedicated Primary Care Physician (MD/DO) to join our team and deliver high-quality care to patients of all ages. If you are committed to preventive care, managing chronic conditions, and providing patient-centered treatment, we encourage you to apply!

Key Responsibilities:

Deliver comprehensive care including preventive services, chronic disease management, and acute care.
Conduct thorough physical exams and review patient medical histories to develop personalized treatment plans.
Order and interpret diagnostic tests, and refer patients to specialists as necessary.
Educate patients on health maintenance, disease prevention, and promoting healthy lifestyle choices.
Collaborate effectively with nursing staff and other healthcare professionals for coordinated care.
Maintain accurate and timely documentation in compliance with regulatory standards.

Qualifications:

Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) from an accredited institution.
Valid medical license to practice in Illinois.
Board certification or eligibility in Family Medicine, Internal Medicine, or a related specialty.
Strong clinical skills with a focus on patient-centered care and effective communication.
Ability to thrive in a collaborative, team-oriented environment.
Dedication to continuous professional development and learning.
