Job Summary:
The Physical Therapist (PT) specializing in Home Health (HH) in a day shift setting. This position requires a qualified healthcare professional capable of providing comprehensive physical therapy services within a home health environment. The assignment is designed to support patient recovery and rehabilitation efforts in a community-based setting over a specified period.

Job Details:

• Schedule: Days shift, 8 hours per day, with a minimum of 40 hours per week
• Required Qualifications: Valid licensure as a Physical Therapist, Home Health experience with a minimum of 1 year, flexible to work day shifts, and a minimum of 0 years of experience
• Preferred Qualifications: Prior experience in Physical Therapist Home Health settings preferred
• Float: Float: Float rotation may be required
• Additional Information: The candidate must possess the ability to work independently, demonstrate strong patient communication skills, and provide evidence of current licensure and CPR certification. Responsibilities include conducting evaluations, developing treatment plans, and implementing therapy interventions tailored to individual patient needs.
