Responsibilities
- Evaluate residents' physical therapy needs and develop individualized treatment plans.
- Deliver a range of PT interventions, including gait training, therapeutic exercises, balance training, and mobility assistance.
- Reassess patient progress, adjust care plans, and document outcomes in accordance with facility policy.
- Collaborate with the interdisciplinary team to support discharge planning and goal attainment.
- Ensure safety and compliance with relevant regulations, maintaining accurate and timely documentation.
- Communicate with residents and families to explain treatment goals and progress, while promoting evidence-based practice.

Requirements
- Current, active Physical Therapist license in California (or eligibility to practice in the state)
- Graduate from an accredited physical therapy program
- Ability to work in a skilled nursing/geriatric rehabilitation setting
- Minimum 35 hours per week available for a travel assignment; flexibility for schedule as needed
- Reliable, patient-centered approach with strong communication and teamwork skills
