Another Chance Programs in San Tan Valley, AZ is looking for a compassionate and dedicated Direct Care Worker to join their team. This is a meaningful opportunity to make a real difference in the lives of individuals who need support, guidance, and care every day. If you have a heart for helping others and a passion for person-centered care, we want to hear from you! Responsibilities:
- Provide direct care and daily living support to clients in a residential or community-based setting
- Assist individuals with personal hygiene, grooming, meal preparation, and daily routines
- Implement individualized care plans and document client progress accurately
- Support clients in developing life skills, independence, and community integration
- Monitor and report changes in client behavior, health, or well-being to supervisors
- Transport clients to appointments, activities, and community outings as needed
- Foster a safe, supportive, and positive environment for all individuals in care
- High school diploma or GED required
- Previous experience in direct care, caregiving, or human services preferred
- Valid driver's license and reliable transportation
- Ability to pass a background check and fingerprint clearance
- Strong communication, patience, and interpersonal skills
- CPR/First Aid certification (or willingness to obtain)
- Genuine compassion and commitment to person-centered care
- Ability to pass a pre-employment drug screening
